IMPORTANT Additional charges may apply for certain treatments. Be sure the print and follow all instructions on the sheet. Sign and date the paperwork and leave it in a place for the technician to easily find (typically the kitchen). Any notes or questions you may have should be written on the sheet by you. The technician will notate any information concerning the treatment process on the sheet and either leave it in your apartment or with the rental office.

If you are a resident all request must go through the leasing office per their requirements and ours. To speed that process up it is recommended to print and fill out the necessary sheet above and bring in the office as it is usually required to schedule most extra treatments.
